ग्रामीण आरोग्य केंद्र खोलने हेतु अर्हताएं एवं नियमावली

केंद्र दुकान का क्षेत्रफल निवेश पूंजी आवेदन शुल्क (नॉन रिफंडेबल)
ग्रामीण आरोग्य केंद्र न्यूनतम 100 - 200 वर्ग फिट 2.5 से 3 लाख रुपये ₹ 9,000/- पतंजलि फूड्स लिमिटेड के नाम इंदौर पर देय।
₹ 9,000/- पतंजलि आयुर्वेद लिमिटेड के नाम हरिद्वार पर देय।
नोट - आवेदक उपरोक्त धनराशि अपने नजदीकी PNB की शाखा में जाकर जमा कर सकता है या Phone Pay, Google Pay,Paytm के माध्यम से मुख्यालय हरिद्वार को दिए गये अकाउंट में जमा कर सकता है ।
  • केंद्र संचालक को पतंजलि आयुर्वेद लिमिटेड के पक्ष में 9000 रुपये और पतंजलि फूड्स के पक्ष में 9000 रुपये का आवेदन शुल्क जमा करना अनिवार्य है, यदि केंद्र संचालक ग्रामीण आरोग्य केंद्र के कोड निर्माण तिथि से लगातार 6 महीने तक अपने पतंजलि ग्रामीण आरोग्य केंद्र के लिए पतंजलि महावितरक (SD) से विधिवत बिलिंग करता है तो ऐसी स्थिति में आवेदक द्वारा मुख्यालय को जमा किये गये आवेदन शुल्क को जमानत राशि में परिवर्तित कर दिया जायेगा । इस 6 माह की अवधि के बाद यदि भविष्य में केंद्र संचालक अपने केंद्र को किन्ही कारणों से बंद करता है तो यह आवेदन शुल्क से जमानत राशि में परिवर्तित धनराशि प्रस्तावित समझौते के नियमों और शर्तों के अनुसार ग्रामीण आरोग्य केंद्र संचालक को वापस कर दी जाएगी ।
  • केंद्र संचालक के लिए केंद्र कोड निर्माण तिथि से एक महीने के भीतर पतंजलि ग्रामीण आरोग्य केंद्र का संचालन व बिलिंग शुरू करना अनिवार्य है । यदि केंद्र संचालक केंद्र कोड निर्माण होने के एक महीने के भीतर पतंजलि ग्रामीण आरोग्य केंद्र का संचालन शुरू करने में विफल रहता है तो ऐसी स्थिति में केंद्र संचालक द्वारा भुगतान किये गये आवेदन शुल्क किसी भी परिस्थिति में जमानत राशि में परिवर्तित नहीं किया जायेगा तथा आवेदन शुल्क को जब्त कर लिया जायेगा एवं वह किसी भी स्थिति में वापिस नहीं किया जायेगा । ऐसी स्थिति में पतंजलि स्वदेशी विभाग हरिद्वार उस स्थान पर दूसरे आवेदक को ग्रामीण आरोग्य केंद्र देने के लिए स्वतंत्र रहेगा ।
  • यदि पतंजलि आयुर्वेद लिमिटेड या पतंजलि फूड्स द्वारा केंद्र संचालक के आवेदन को खारिज कर दिया जाता है तो ऐसी स्थिति में केंद्र संचालक को पतंजलि स्वदेशी विभाग द्वारा पूर्ण आवेदन शुल्क अस्वीकृति की तारीख से 15 कार्य दिवसों के भीतर वापस की जायेगी ।
  • केन्द्र संचालक को ग्रामीण आरोग्य केन्द्र पर मुख्यालय से निर्धरित लगभग 3000 हजार रूपये के डिस्प्ले बोर्ड का खर्च स्वयं व्यय करना होगा।
  • ग्रामीण आरोग्य केन्द्र पर पतंजलि उत्पाद रखने के लिए लकड़ी/स्टील रेक की व्यवस्था का खर्च संचालक को स्वयं व्यय करना होगा एवं मुख्यालय के बिना अनुमति के रैक एवं साइन बोर्ड न बनवाएँ।
  • आवेदनकर्ता आवेदन करते समय स्थान (दुकान) की अंदर व बाहर से तथा मार्केट की फोटो साथ में जरूर उपलब्ध् कराएँ।
  • आवेदनकर्ता का आवेदन संस्थान द्वारा निर्धारित न्यूनतम दूरी के मापदण्डों के आधर पर ही स्वीकृत किया जाएगा।
    (नोट: आवेदन करते समय आवेदक स्थान का चयन पतंजलि आयुर्वेद द्वारा वर्तमान संचालित केन्द्र से न्यूनतम 2.5 कि.मी. की दूरी पर ही चयन करें-)
  • आवेदक पर न्यायालय से सम्बंधित किसी भी प्रकार का वाद-विवाद नहीं होना चाहिए।
  • हरिद्वार मुख्यालय से अनुमति के पश्चात ही केन्द्र के लिए दुकान का किराया अनुबंध (Rent Agreement) करें।
  • पतंजलि ग्रामीण आरोग्य केन्द्र गाँव के प्राइम लोकेशन व प्रमुख बाजार में ही खोले जायेंगे।
  • पतंजलि ग्रामीण आरोग्य केन्द्र पर पतंजलि फूड्स, पतंजलि आयुर्वेद व संस्थान द्वारा अनुमोदित वस्तुओं के अतिरित्तफ किसी भी अन्य कम्पनी के उत्पाद विक्रय करना पूर्णतया प्रतिबंधित है।
  • आवेदन के पश्चात आवेदन की स्वीकृति सम्बंधित विभाग के माध्यम से नियमानुसार जाँच के पश्चात ही होगी।
    (नोट-इस सम्बन्ध में आवेदक द्वारा किसी भी प्रकार का दावा स्वीकार नहीं किया जाएगा)
  • नशीले पदार्थों का व्यापार करने वाले व्यक्ति आवेदन-पत्र न भरें क्योंकि हमारी संस्था एक सामाजिक और आध्यात्मिक संस्था है।

Qualifications and Rules for Opening Gramin Arogya Kendra

Note - The applicant can deposit the above amount by visiting his nearest PNB branch or by phone. Pay can be deposited in the account given to the headquarters Haridwar through Google Pay, Paytm.
  • It is mandatory for the center operator to deposit an application fee of Rs. 9000 in favor of Patanjali Ayurveda Limited and Rs. 9000 in favor of Patanjali Foods, if the center operator has registered with Patanjali for his Patanjali Gramin Arogya Kendra for 6 consecutive months from the date of code creation of the Gramin Arogya Kendra. If the General Distributor (SD) does the billing properly, then in such a situation the application fee deposited by the applicant to the Headquarters will be converted into security amount. After this period of 6 months, if in future the center operator closes his center due to any reason, then the amount converted from application fee into security deposit will be returned to the Gramin Arogya Kendra operator as per the terms and conditions of the proposed agreement.
  • It is mandatory for the center operator to start operation and billing of Patanjali Gramin Arogya Kendra within one month from the date of creation of the center code. If the center operator fails to start the operation of Patanjali Gramin Arogya Kendra within one month of the creation of the center code, then in such a situation the application fee paid by the center operator will not be converted into security amount under any circumstances and the application fee will not be converted into security amount. It will be confiscated and will not be returned under any circumstances. In such a situation, Patanjali Swadeshi Department, Haridwar will be free to give the Gramin Arogya Kendra to another applicant at that place.
  • If the Center Operator's application is rejected by Patanjali Ayurveda Limited or Patanjali Foods, then in such a situation the full application fee will be refunded to the Center Operator by Patanjali Swadeshi Department within 15 working days from the date of rejection.
  • The center operator will have to bear the cost of display board of about Rs. 3000 thousand fixed by the headquarters at the Gramin Arogya Kendra.
  • The operator will have to bear the cost of arranging wooden/steel rake to keep Patanjali products at the Gramin Arogya Kendra and should not make racks and sign boards without permission from the headquarters.
  • While applying, the applicant must provide the inside and outside photographs of the place (shop) and the market along with it.
  • The applicant's application will be approved only on the basis of minimum distance criteria set by the institute.
    (Note: While applying, the applicant should select the place at a minimum distance of 2.5 km from the center currently operated by Patanjali Ayurveda -)
  • The applicant should not have any kind of dispute related to the court.
  • Enter into the rent agreement for the shop for the center only after permission from the Haridwar headquarters.
  • Patanjali Gramin Arogya Kendras will be opened in prime locations and major markets of the village only.
  • Selling products of any other company other than Patanjali Foods, Patanjali Ayurved and the items approved by the institute is completely prohibited at Patanjali Gramin Arogya Kendra.
  • People involved in drug dealing should not fill the application form because our organization is a social and spiritual organization.

ग्रामीण आरोग्य केंद्र प्रारम्भ करने हेतु आवश्यक दस्तावेज

  • आवेदन फॉर्म, मुखिया/सरपंच पत्र, दुकान की फोटो
  • आवेदक का आधार कार्ड, पैन कार्ड, Passport Size फोटो, फ़ूड लाइसेंस (FSSAI Licence), बाट व माप- (Weights & Measures) प्रमाणपत्र, कैंसिल चेक या बैंक स्टेटमेंट, स्वयं की दुकान है तो उसका स्वामित पत्र या बिजली बिल (स्वयं के नाम, माता-पिता या पत्नी के नाम से) , दुकान किराए की है तो किरायानामा ।
  • यदि फर्म (Firm) के नाम से एग्रीमेंट करना है तो सभी दस्तावेज़ फर्म (Firm) के नाम से बनवाएं और यदि एग्रीमेंट आवेदक अपने नाम से करना चाहता है तो सभी दस्तावेज़ अपने नाम से बनाएं ।
  • अनुबंध (Agreement) के समय फर्म (Firm) के नाम का मोहर साथ लेकर आयें । अनुमति लेना अनिवार्य है ।
    • नोट :- आवेदक अपने फर्म का नाम बनाने में या GST बनाने में "पतंजलि" नाम या "ग्रामीण आरोग्य केंद्र" के नाम से न बनाएं ऐसा करने पर आपका अनुबंध (Agreement) नहीं हो पाएगा । आवेदक को अनुबंध (Agreement) हेतु हरिद्वार आने से पूर्व मुख्यालय का

ग्रामीण आरोग्य केन्द्र मे प्रदान की जाने वाली सुविधाएं :-

  • ग्रामीण आरोग्य केंद्र के लिए उत्पादों की Supply पतंजलि महावितरक (SD) द्वारा नि:शुल्क की जाती है ।
  • ग्रामीण आरोग्य केंद्र के लिए महावितरक (Super Distributor) से प्रथम बिलिंग न्यूनतम 2 लाख अनिवार्य है, उसके बाद केंद्र संचालक आगे के आर्डर बिलिंग न्यूनतम 25000/- रूपये तक कर सकता है ।
  • ग्रामीण आरोग्य केंद्र संचालक महावितरक से पतंजलि आयुर्वेद व पतंजलि फूड्स के कुल 1600 से अधिक उत्पादों का Order क्षेत्र में मांग के अनुसार लगाने के लिए स्वतंत्र है ।

    वैध/चिकित्सक परामर्श सुविधा –

    ग्रामीण आरोग्य केंद्र पर उपचार हेतु आने वाले मरीजों को मुख्यालय हरिद्वार से प्रत्येक सप्ताह एक दिन पतंजलि के चिकित्सक (Doctor’s) द्वारा निशुल्क ऑनलाइन (online) चिकित्सा परामर्श दिया जाता है ।

    POS बिलिंग मशीन सुविधा –

    पतंजलि POS Machine प्रत्येक ग्रामीण आरोग्य केंद्र को लगाना अनिवार्य है इस Machineसे ग्रामीण आरोग्य केंद्र संचालक को निम्न सुविधाएँ प्रदान की जाती हैं –

  • ग्राहक द्वारा खरीदे गये सामान का बिल भुगतान व अन्य स्टॉक Inventory Management का कार्य POS Machine द्वारा किया जाता है |
  • ग्राहक यदि समृद्धि कार्ड, ATM कार्ड या पतंजलि क्रेडिट कार्ड से खरीदारी करता है तो कार्ड Swipe का कार्य POS Machine द्वारा किया जाता है ।
  • POS Machine की वास्तविक कीमत 22000/- रूपये है, जिसे केंद्र संचालक को संस्थान द्वारा निःशुल्क उपलब्ध कराया जाता है, लेकिन POS Billing Machine के Software रख-रखाव (Maintanence) हेतु ग्रामीण आरोग्य केंद्र संचालक को मासिक 500/- रूपये संस्था को भुगतान करना अनिवार्य है । POS Billing Machine लगाने पर केंद्र संचालक को अपने केंद्र पर बिलिंग हेतु अलग से कंप्यूटर एवं प्रिंटर व्यवस्था करने की आवश्यकता नहीं है ।

    पतंजलि क्रेडिट कार्ड सुविधा –

  • पतंजलि आयुर्वेद अपने ग्रामीण आरोग्य केंद्र संचालक एवं ग्राहकों को पंजाब नेशनल बैंक (PNB) के सहयोग से अति सरल प्रक्रिया के तहत क्रेडिट कार्ड उपलब्ध करता है ।
  • इस क्रेडिट कार्ड में 25000/- से 500000/- तक कैश की लिमिट उपलब्ध रहती है एवं इस क्रेडिट कार्ड के द्वारा ग्राहक 49 दिन तक बिना ब्याज के उधारी में भी उत्पाद खरीद सकता है ।
  • क्रेडिट कार्ड के द्वारा ग्राहक एवं संचालक दोनों को 2% कैशबैक व अन्य व्यापारिक ऑफर उपलब्ध करता है एवं कार्डधारकों को 2 लाख से 10 लाख का दुर्घटना बीमा उपलब्ध रहता है ।
  • क्रेडिट कार्ड में कार्डधारकों को अधिकतम 49 दिन में क्रेडिट राशि को वापस भुगतान करने का समय मिलता है ।

    • व्यापार-विस्तार हेतु ऋण (Loan) सुविधा -

    ग्रामीण आरोग्य केंद्र संचालक यदि अपने ग्रामीण आरोग्य केंद्र को प्रथम 3 माह तक व्यवस्थित रूप से संचालित करता है तो पतंजलि आयुर्वेद के साथ व्यापार विस्तार हेतु पतंजलि एवं SIDBI के सहयोग से केंद्र संचालक को न्यूनतम ब्याज दर पर 50 हजार से 5 लाख तक व्यापार ऋण (Business Loan) दिया जाता है । यह ऋण केंद्र संचालक को अधिकतम 3 वर्ष तक मासिक या त्रिमासिक क़िस्त में वापस अदा करना अनिवार्य है ।

    Expire & Damage Policy –

    ग्रामीण आरोग्य केंद्र में यदि कोई उत्पाद Expire & Damage होता है, तो उसे वापस किया जाता है । शर्तें लागू (T&C Apply)

    ग्राहक के लिए स्वदेशी समृद्धि कार्ड की सुविधा –

    स्वदेशी समृद्धि कार्ड पतंजलि आयुर्वेद का एक शॉपिंग कार्ड है । यह समृद्धि कार्ड केंद्र संचालक को प्रति कार्ड 90/- रूपये में पतंजलि आयुर्वेद हरिद्वार द्वारा उपलब्ध कराया जाता है । ग्रामीण आरोग्य केंद्र से ग्राहक का समृद्धि कार्ड मात्र 100/-रूपये में बनाया जाता है । स्वदेशी समृद्धि कार्ड की योजना से केंद्र संचालक को स्थाई (Permanent) ग्राहक मिलते हैं एवं इस कार्ड से खरीदारी करने पर ग्राहक को निम्न सुविधाएं प्रदान की जाती हैं -

  • स्वदेशी समृद्धि कार्ड से खरीदारी करने पर ग्राहक को ग्रामीण आरोग्य केंद्र से उत्पादों पर 5% से 10% तक की छूट (Cash Back) मिलता है ।
  • केंद्र संचालक यदि एक माह के अवधि में 15 स्वदेशी समृद्धि कार्ड बनाता है तो प्रत्येक स्वदेशी समृद्धि कार्ड पर केंद्र संचालक को 50 रूपये दिये जाते हैं और यदि केंद्र संचालक 15 से अधिक स्वदेशी समृद्धि कार्ड बनाता है तो प्रत्येक स्वदेशी समृद्धि कार्ड पर केंद्र संचालक को 100 रूपये प्रोत्साहन राशि पतंजलि आयुर्वेद द्वारा दिया जाता है ।
  • स्वदेशी समृद्धि कार्ड से खरीदारी करने पर ग्राहक को ग्रामीण आरोग्य केंद्र से आकस्मिक दुर्घटना में मृत्यु होने पर 5 लाख तथा दिव्यांग होने पर 2.5 लाख का दुर्घटना बीमा दिया जाता है । भारत में ग्राहक को इस प्रकार की योजना देने वाली पतंजलि एक मात्र संस्थान है ।
  • स्वदेशी समृद्धि कार्ड केंद्र संचालक को मुख्यालय हरिद्वार से उपलब्ध कराये जाते हैं । स्वदेशी समृद्धि कार्ड के लिए केंद्र संचालक को मुख्यालय हरिद्वार द्वारा दिए गये बैंक खाते में 10000/- हजार रूपये जमा करना है । उपरोक्त धनराशि (Payment) आवेदक स्वयं अपने नजदीकी PNB की शाखा में जाकर जमा कर सकता है या Phone Pay, Google Pay या Paytm के माध्यम से मुख्यालय हरिद्वार को दिए गये अकाउंट में जमा कर सकता है ।

Documents required for starting a Gramin Arogya Kendra

  • Application form, Head/Sarpanch Letter, Photo of Shop
  • Applicant's Aadhar Card, PAN Card, Passport Size Photo, Food License (FSSAI License), Weights & Measures Certificate, Canceled Check or Bank Statement, If you have your own shop, then its Ownership Letter or Electricity Bill (for your own Name, Name of Parents or Wife), rent deed if the shop is on rent.
  • If the Agreement is to be made in the name of a firm, then make all the Documents in the name of the firm and if the Applicant wants to make the Agreement in his own Name, then make all the Documents in his own Name.
  • At the time of Agreement, bring the seal of the firm's Name with you. It is mandatory to take permission.
    • Note:- Applicants should not use the Name "Patanjali" or "Gramin Arogya Kendra" while making the name of their firm or while making GST, otherwise your agreement will not be possible. The applicant should visit the Headquarters before coming to Haridwar for the Agreement.

Facilities Provided in Gramin Arogya Kendra:-

  • The Products for the Gramin Arogya Kendra are supplied free of cost by Patanjali Mahavitarak (SD).
  • For Gramin Arogya Kendra, the first billing from the Super Distributor is mandatory, minimum Rs 2 lakh, after that the center operator can make further billing orders up to a minimum of Rs 25000/-.
  • Gramin Arogya Kendra operators are free to order more than 1600 products of Patanjali Ayurveda and Patanjali Foods from Mahadistributor as per the demand in the area.

    Legal/Medical Consultation facility –

    Patients coming for treatment at the Gramin Arogya Kendra are given free online Medical consultation by Patanjali's Doctors from the Headquarter Haridwar once every week.

    POS Billing Machine Facility –

    It is mandatory to install Patanjali POS Machine in every Gramin Arogya Kendra. With this machine, the following facilities are provided to the Gramin Arogya Kendra Operator –

  • The work of bill payment and other stock inventory management of goods purchased by the customer is done by POS machine.
  • If a customer makes a purchase through Samriddhi Card, ATM Card or Patanjali Credit Card, then the work of card swipe is done by the POS Machine.
  • The actual price of the POS machine is Rs 22000/-, which is provided free of cost by the institute to the center operator, but for the software maintenance of the POS Billing Machine, the Gramin Arogya Kendra operator has to pay Rs 500/- monthly to the institute. mandatory . By installing POS Billing Machine, the center operator does not need to arrange for a separate computer and printer for billing at his center.

    Patanjali Credit Card Facility –

  • Patanjali Ayurveda provides credit cards to its Gramin Arogya Kendra operators and customers through a very simple process in collaboration with Punjab National Bank (PNB).
  • In this credit card, cash limit is available from Rs 25000/- to Rs 500000/- and through this credit card the customer can also buy products on credit without interest for 49 days.
  • Through credit card, 2% cashback and other business offers are available to both the customer and the operator and accident insurance of Rs 2 lakh to 10 lakh is available to the cardholders.
  • In credit cards, cardholders get a maximum of 49 days to pay back the credit amount.

    • Loan facility for Business Expansion -

    If the Gramin Arogya Kendra operator runs his Gramin Arogya Kendra systematically for the first 3 months, then with the help of Patanjali and SIDBI, the center operator can get a business loan of Rs 50 thousand to Rs 5 lakh at the lowest interest rate for business expansion with Patanjali Ayurveda. Business Loan is given. It is mandatory to repay this loan to the center operator in monthly or quarterly installments for a maximum of 3 years.

    Expire & Damage Policy –

    If any Product expires and is damaged in the Gramin Arogya Kendra, it is returned. Terms Apply (T&C Apply)

    Facility of Swadeshi Samriddhi Card for the customer –

    Swadeshi Samriddhi Card is a shopping card of Patanjali Ayurved. This Samriddhi Card is provided to the center operator by Patanjali Ayurveda Haridwar for Rs. 90/- per card. Customer's Samriddhi Card is made from Gramin Arogya Kendra for just Rs 100/-. Through the scheme of Swadeshi Samriddhi Card, the center operator gets permanent customers and on making purchases through this card, the following facilities are provided to the customer -

  • On purchasing through Swadeshi Samriddhi Card, the customer gets 5% to 10% discount (Cash Back) on products from Gramin Arogya Kendra.
  • If the center operator makes 15 Swadeshi Samriddhi cards in a period of one month, then Rs. 50 is given to the center operator on each Swadeshi Samriddhi card and if the center operator makes more than 15 Swadeshi Samriddhi cards, then Rs. 100 is given to the center operator on each Swadeshi Samriddhi card. The incentive amount is given by Patanjali Ayurveda.
  • On purchasing through Swadeshi Samriddhi Card, the customer is given accident insurance of Rs 5 lakh in case of death due to accidental accident and Rs 2.5 lakh in case of disability from Gramin Arogya Kendra. Patanjali is the only organization in India to offer this type of scheme to its customers.
  • Swadeshi Samriddhi cards are made available to the center operator from the headquarters, Haridwar. For Swadeshi Samriddhi Card, the center operator has to deposit Rs 10,000/- in the bank account given by the headquarters, Haridwar. The applicant can deposit the above amount (payment) himself by visiting his nearest PNB branch or can deposit it in the account given to the headquarter Haridwar through Phone Pay, Google Pay or Paytm.
